A Review of 2025 Remote Therapeutic Monitoring (RTM) CPT Codes
Every calendar year, the Centers for Medicare & Medicaid Services (CMS) revises its CPT billing and reimbursement codes for healthcare providers. These changes can affect primary care offices in a variety of ways. For those who use remote therapeutic monitoring CPT codes, it’s wise to stay sharp about any changes ahead. Let’s take a look at some of the updates coming to virtual healthcare in 2025, including any changes to CPT codes and reimbursement policies.
What is Remote Therapeutic Monitoring (RTM)?
Remote therapeutic monitoring, or RTM for short, are approved medical devices that collect and transmit patient data in real time. RTM specifically gathers non-physiological information for clinicians to review as their patients go about their daily lives. They allow for more targeted adjustments to behavioral treatments.
RTM devices can transmit the following:
- Musculoskeletal data
- Respiratory data
- Therapy response
- Therapy adherence
RTM vs. RPM: Key Differences
Remote patient monitoring, or RPM, diverges from RTM in two important ways. First, patients cannot self-report data with an RPM device, while they can with RTM. Second, RPM devices gather and transmit physiological data. They measure a patient’s vitals and more, such as:
- Blood pressure
- Blood oxygen level
- Prescription response
- Prescription adherence
Because RTM and RPM are used for different kinds of treatment, they have to be billed differently. Unlike RPM, RTM codes fall under general medicine. This means occupational and physical therapists can bill for them too.
CMS Remote Therapeutic Monitoring Updates
While the 2025 RTM codes have not changed in the CMS system, there was one important update. Due to public comments and requests, CMS is finalizing a proposal to allow concurrent billing for RTM and RTM services and other advanced primary care management (APCM) services.
This means RTM codes will be billable in the same month as codes for:
- RPM
- BHI
- CHI
- PIN
- PIN-PS
- SDOH Risk Assessment.
2025 Remote Therapeutic Monitoring CPT Codes & Reimbursement
The 2025 RTM CPT codes are the same as those from 2024, but we’ll list them for easy access.
One note to keep in mind is that each of these codes requires at least 16 days of data collection within a 30-day billable period. Additionally, providers can find their localized billing/reimbursement rate for each code using the Medicare Physician Fee Schedule Lookup Tool.
